复发性流产患者免疫治疗前后外周血T淋巴细胞表达率的变化及疗效研究

目的 观察复发性流产(RSA)患者免疫治疗前后外周血T淋巴细胞表达率的变化情况及临床治疗效果.方法 选取2013年1月至2014年2月110例封闭抗体阴性复发性流产患者,采用双荧光标记流式细胞分析技术,以健康男性或其丈夫淋巴细胞作为抗原靶标,全部RSA患者均接受免疫治疗,对治疗前后CD3+、CD4+、CD8+细胞表达率进行分析,并观察分析妊娠结局,总结临床治疗效果.结果 治疗后患者外周血中CD3+、CD4+、CD8+值高于治疗前(P＜0.05);治疗前后,未转阳、转阳患者CD3+、CD4+、CD8+值差异有统计学意义,治疗后转阳患者高于未转阳者;治疗后,未转阳82例,成功妊娠率为15.85％(13/82),转阳28例,成功妊娠率为35.71％(10/28),差异有统计学意义(P＜0.05).结论 对复发性流产患者进行免疫治疗,可调控母胎界面免疫耐受形成,提高成功妊娠率,提升患者生活质量,值得临床推广应用.

Study on the change of expression rate of T Lymphocyte in the peripheral blood of patients with recurrent spontaneous abortion before and after immunological therapy and the clinical therapeutic effect

Objective To observe the change of expression rate of T Lymphocyte in the peripheral blood of patients with recurrent spontaneous abortion (RSA) before and after immunological therapy and the clinical therapeutic effect.Methods 110 closed antibody negative patients with RSA from January 2013 to February 2014 were selected.Double immunofluorescence flow cytometry was adopted and the lymphocyte of healthy male or patients' husband was as the antigen target.All RSA patients received immunological therapy.The expression rate of CD3+,CD4+,CD8+ cells before and after treatment were analyzed.The pregnancy outcomes were observed and analyzed and the clinical curative effect was concluded.Results CD3+,CD4+,CD8+ values in the peripheral blood of patients after treatment were higher than those before treatment (P＜0.05);there were statistically significant differences in CD3+,CD4+,CD8+ values of not-turning-positive and turningpositive patients before and after treatment,those of turning-positive patients after treatment were higher than those of not-turning-positive patients;after treatment,the successful pregnancy rate was 15.85％ (13/82) in 82 cases of not-turning-positive,that was 35.71％ (10/28) in 28 cases of turning-positive,with statistically significant difference (P＜0.05).Conclusion Immunological therapy in patient with RSA can regulate the formation of maternal-fetal interface immune tolerance,improve the pregnancy rate and the living quality of patients,worthy of clinical application and popularization.
