肝郁脾虚型肠易激综合征便秘大鼠模型的建立和评价

摘    要:目的:筛选便秘型肠易激综合征动物模型的最佳建立方法,并探讨所建立模型的中医证候属性。方法:分别采用母婴分离法、冰水灌胃法、限水法、母婴分离结合冰水灌胃法、母婴分离结合限水法制作便秘大鼠模型,观察大鼠一般行为学变化、体重和24h进食量,测定4h内排便颗粒数,并对大鼠粪便进行Bristol分型积分和含水量测定,采用直肠扩张实验评价内脏敏感性,观察各组大鼠肠组织形学变化,筛选出符合IBS-C临床特征的动物模型建立方法。并采用复方中药逍遥散进行反证,判定模型的中医证候范畴。结果:采用母婴分离结合冰水灌胃法建立的模型具有排便颗粒数减少,粪便含水量降低, Bristol分型积分显示为便秘型的特征,且其肠道敏感性显著升高,结肠组织无形态学改变,符合IBS-C的临床特征。经逍遥散验证,该IBS-C模型属于中医“肝郁脾虚证”范畴。结论:母婴分离结合冰水灌胃法能够建立中医肝郁脾虚证IBS-C大鼠模型,能高度拟合IBS-C的临床特征,造模成功率高。

Abstract:Objective: To screening the methods of establishing constipation-predominant irritable bowel syndrome (IBS-C) rat model, and analyze the TCM syndrome differentiation of the IBS-C model. Methods: Using maternal separation (MS), ice water stimulation, water limitation, MS combined with ice water stimulation, MS combined with water limitation to establish 5 kinds of constipation model. Observing the general behavior, body weight and daily food-intake and measuring defecation numbers and water content of the defecations in 4hrs, as well as making a classification by Bristol integral. The colorectal distention test was used to assess the intestinal sensitivity, and haemotoxylin staining was used to observe the form of organization of colons. Results: MS combined with ice water stimulation could induce a rat model with defecation numbers decreased, water content of defecation decreased. Meanwhile, the classification by Bristol integral showed that the model was an IBS-C model with viscera hypersensitivity, but there was no obvious change of the form of organization of colons. The IBS-C model belonged to liver-stagnation and spleen-deficiency syndrome when testified by Xiaoyao powder. Conclusions: MS combined with ice water stimulation was the best methods to establish an IBS-C rat model with liver-stagnation and spleen-deficiency, which has the high degree to match its clinical symptoms.
