应用ROC曲线评价超敏肌钙蛋白T对急性心肌梗死的诊断价值及诊断界值的确定

目的 通过绘制超敏肌钙蛋白T(cTnT)和肌钙蛋白T(cTnT)对于急性心肌梗死(AMI)与稳定型心绞痛(SAP)鉴别诊断的ROC曲线,对比分析两种方法的优越性,确定本实验室的诊断界值.方法 电化学免疫发光法测定AMI组、SAP组与健康对照组的血清超敏cTnT和cTnT,并对超敏cTnT和cTnT进行统计学比较(t检验)和ROC分析.结果 超敏cTnT在AMI诊断中的敏感度为82.6％,特异度为92.3％,阳性结果预测值为97.9％,诊断临界值为0.035 ng/mL;cTnT的敏感度为78.3％,特异度为84.6％,阳性结果预测值为94.4％,诊断临界值为0.079 ng/mL.超敏cTnT的ROC曲线下面积(AUC)为0.916,cTnT的AUC=0.856.结论 血清超敏cTnT是诊断急性心肌梗死灵敏而特异的诊断指标,其诊断性能优于cTnT.

Application ROC curves contrast analysis cTnT and cTnT allergic the diagnostic value of myocardial infarction

Objective Ren Tintin,Deng Song,Wang Jinzhi.Department of Biochemistry,The Affiliated Hospital of Guiyang Medical College,Guiyang 550004,China.Methods measured the calm stem group,the stable angina pectoris group with healthy control group’s blood serum ultra sensitive cTnT and cTnT,and carry on statistics comparison to ultra sensitive cTnT and cTnT(the T-test) and the ROC analysis by electrochemistry immunity illumination law.Results Finally ultra sensitive cTnT diagnoses at the cardiac arrest the sensitivity was 82.6%,the peculiarity was 92.3%,the masculine result forecast value was 97.9%,the diagnosis marginal value was 0.035ng/mL.The cTnT sensitivity was 78.3%,the peculiarity was 84.6%,the masculine result forecast value was 94.4%,the diagnosis marginal value was 0.079ng/mL.Under ultra sensitive cTnT ROC curve area AUC= 0.916,cTnT AUC = 0.856.Conclusion Blood serum ultra sensitive cTnT is the diagnosis acute cardiac arrest keen and the special diagnosis target,its diagnosis performance surpasses cTnT obviously,has the good clinical diagnosis performance as the diagnosis acute cardiac arrest assistance diagnosis target.
