结核性广泛粘连性小肠梗阻的手术处理

目的:探讨结核性广泛粘连性小肠梗阻(TBEAIO)的手术时机、方法及再梗阻的预防。方法:对8例TBEA IO的临床资料进行回顾性分析,并对其手术操作的可行性进行研究。结果:8例中男2例,女6例,平均年龄31.6岁,皆有典型小肠梗阻表现,经手术与术后病理证实为本病。7例行肠粘连松解、肠排列术,术中予以抗粘连预防,均获得满意疗效,1例就诊过晚,仅行破裂肠袢外引流,抢救无效死亡。结论:对TBEAIO应遵循手术指征,正确施行肠粘连松解术,做好肠袢再粘连、再梗阻的预防。

Surgical treatment of tuberculous and extensively adhesive intestinal obstruction

Objective: To explore the operation timing,methods and prevention of reobstruction for tuberculosus and extensively adhesive intestinal obstruction(TBEAIO).Methods: Clinical data of 8 cases with TBEAIO in our hospital in recent 10years were analyzed retrospectively to investigate its feasible surgical procedures.Results: Among the 8 patients,2 males and 6 females,mean age was 31.6years and all patients had typical characteristic of small bowel obstruction.The diagnosis was proved by operation and operation.7 patients recovered after adhesion was 1ysed,intestin was rear ranged.One patient was forced to receive intestinal perforation drainage because of visiting hospital too late and died finally.Conclusion:Intestinal adheson and prevention of readhesive obstruction of are the best procedures for TBEAIO.
