49例肾门淋巴结炎性增大的T_(1-3a)N_0M_0肾细胞癌患者的临床分析

背景与目的：合并肾门淋巴结炎性增大的T1-3aN0M0肾细胞癌患者经临床治疗后,其预后未见系统性报道。本研究拟调查T1-3aN0M0肾细胞癌合并肾门淋巴结炎性增大患者的预后并做临床分析。方法：查阅并记录1995年1月—2000年1月入院治疗且病理确诊的49例淋巴结炎性增大的T1-3aN0M0肾细胞癌患者的病历资料,通过电话和平信随访其预后。结果：随访8～14年,平均7.5年。失访7例;无瘤健在17例;带瘤存活4例;死亡21例。死于肾癌相关并发症18例,死于心脑血管病3例。手术1年后肺或胸壁转移8例、同侧肾门淋巴结转移1例;2年后左侧胫骨上端转移1例、左侧肱骨上端转移1例;3年后肺转移5例、腹部皮肤转移2例;5年后肺或颈部淋巴结或皮肤转移4例。本组肾细胞癌5年和10年存活率分别为53.1%（26/49）和42.9%（21/49）。结论：肾细胞癌中大多数肾门增大淋巴结为淋巴结炎。合并肾门淋巴结炎性增大的T1-3aN0M0肾细胞癌患者的5年和10年长期存活率较低。区域淋巴结清扫或炎性增大淋巴结切除术不能完全防止T1-3aN0M0肾细胞癌远处转移的发生。建议对于合并单纯肾门淋巴结炎性增大的肾细胞癌可行包括增大淋巴结在内的局限性淋巴结切除术。
