动态血糖监测联合胰岛素泵在围手术期糖尿病患者中的应用

摘    要:目的 :探讨动态血糖监测联合胰岛素泵应用于围手术期糖尿病患者血糖控制的效果。方法 :选取2011年1月至2013年2月我院收治的100例围手术期糖尿病患者,根据血糖监测方案不同分为2组,对照组采用每日8次血糖监测方案,观察组采取动态血糖监测,对2组患者动态血糖监测、输注胰岛素前和围手术期的各项数据进行统计分析。结果:动态血糖监测、输注胰岛素前2组患者的体质量指数、空腹血糖、2 h血浆葡萄糖及糖化血红蛋白水平差异无统计学意义(P>0.05),而在血糖达标时间、胰岛素用量、住院时间及术后并发症发生率对比上,动态血糖监测组均显著优于对照组,差异有统计学意义(P<0.05)。动态血糖监测组的血糖控制情况亦显著优于对照组,差异有统计学意义(P<0.05)。结论:动态血糖监测可提高围手术期糖尿病患者的血糖控制效果,缩短血糖达标时间和住院时间,减少胰岛素用量,可更好地控制术后并发症的发生,值得在临床上推广和应用。

Abstract:Objective: To investigate the effect of application of dynamic blood sugar monitoring combined with in- sulin pump in perioperative period of patients with diabetes mellitus. Methods: One hundred patients with diabetes melli- tus undergoing operation in our hospital during Jan. 2011 to Feb. 2013 were enrolled. Patients were divided into two groups according to the blood glucose monitoring program. The blood glucose level in control group was assayed 8 times daily, and in observation group was measured with dynamic glucose monitoring. Data before treatment and during perioperative period between patients of these two groups were compared. Results: BMI, FPG, 2 h PG and HbAlc before treatment were not significantly different (P〉0.05) between the two groups, while the time of reaching standard blood glucose level, insulin dosage, duration of hospitalization and the incidence of postoperative complication were significantly better in observation group than in control group (P〈0.05). The control of blood glucose in observation group was significantly better than in con- trol group (P〈0.05). Conclusions: For perioperative patients with diabetes mellitus, use of dynamic glucose monitoring with insulin pump could improve blood glucose management, shorten the time of reaching standard blood glucose level and du- ration of hospitalization, reduce the insulin dosage and be beneficial for control of postoperative complication.
