慢性乙型肝炎患者CD8~+CD28~+T/CD8~+CD28~-T比值变化及临床意义

目的探讨慢性乙型肝炎患者CD8+CD28+T/CD8+CD28-T比值变化及临床意义。方法 流式细胞仪测定正常对照、慢性乙型肝炎患者CD8+CD28+T/CD8+CD28-T的百分数,并计算CD8+CD28+T/CD8+CD28-T比值,慢性乙型肝炎组根据HBV-DNA含量分为两组,低病毒载量组(HBV-DNA<106拷贝/ml)和高病毒载量组(HBV-DNA>106拷贝/ml),并分别计算CD8+CD28+T/CD8+CD28-T比值。结果慢性乙型肝炎患者组CD8+CD28+T/CD8+CD28-T比值与正常对照组相比明显降低,两者差异有统计学意义(P<0.01);慢性乙型肝炎高病毒载量组CD8+CD28+T/CD8+CD28-T比值低于低病毒载量组(P=0.039),两组差异有统计学意义。结论 慢性乙型肝炎患者CD8+CD28+T/CD8+CD28-T比值下降可能是病毒不能清除,而长期存在的原因之一。

Clinical significance of the ratio of CD8~+CD28~+ to CD8~+CD28~-T lymphocytes in chronic HBV infection

Objective To explore the clinical significance of the ratio of CD8＋CD28＋ to CD8＋CD28-T lymphocytes in chronic HBV infection.Methods We randomly selected 87 chronic HB patients and 38 healthy individuals for the study.The patients of chronic HB were divided into two groups by HBV DNA concentrations（group 1： 48 patients,HBV-DNA106 copies/ml;group 2：39 patients,HBV-DNA≥106 copies/ml）.The concentrations of CD8＋CD28＋ and CD8＋CD28-T cells were determined by flow cytometry.The ratio of CD8＋CD28＋ to CD8＋CD28-T cells was then calculated.Results The ratio of CD8＋CD28＋ to CD8＋CD28-T cells was significantly lower in chronic HB patients than in health controls（P0.05）.The ratio of CD8＋CD28＋ to CD8＋CD28-T cells was also significantly lower in chronic HB patients with higher viral load（group2） than in chronic patients with lower viral load（group1）（P0.05）.Conclusion A decrease in the ratio of CD8＋CD28＋ to CD8＋CD28-T cells may be associated with the failure of HBV clearance in chronic HB patients.
