摘    要:目的:分析血液与痰标本中分离的鲍曼不动杆菌与肺炎克雷伯菌,掌握鲍曼不动杆菌与肺炎克雷伯菌在血液与痰标本中的差异性,以及这两种细菌的耐药变迁,为临床合理应用抗菌药物提供有力证据。方法:采用回顾性的方法对我院2003年1月至2009年9月血液与痰标本中分离的鲍曼不动杆菌与肺炎克雷伯菌培养和药敏实验的结果进行统计、分析。结果:美洛培南(MEM)、头孢哌酮/舒巴坦(CSL)、左氧氟沙星(LEV)对鲍曼不动杆菌杆菌及肺炎克雷伯菌的保持了良好的抗菌活;头孢菌素类抗菌药物对鲍曼不动杆菌与肺炎克雷伯菌的治疗效果不佳;2003年至2009年两种细菌耐药总体趋势是先快速上升再缓慢下降。结论:临床医生长期大量使用抗生素,使得革兰阴性菌的耐药率逐年升高,但近年来由于医院加强了医院感染的监测,大力宣传抗菌药物的合理应用,耐药率有所下降。

Abstract:Objective To study the drug resistance of Acinetobacter baumanii and Klebsiella pneumoniae isolated from Blood and sputum so as to provide references of rational use of antibiotics in the clinic.Methods The susceptibility results of Acinetobacter baumanii and Klebsiella pneumoniae isolated from blood and sputum in our hospital from January 2003 to September 2009 were analyzed retrospectively.Results Acinetobacter baumannii and Klebsiella pneumoniae were highly sensitive to meropenem(MEM),cefoperazone /sulbactam(CSL) and levofloxacin(LEV);the treatments of cephalosporin antibiotics for infection of Acinetobacter baumannii and Klebsiella pneumoniae were poor,so clinical use should be carefully;the drug resistance of Acinetobacter baumanii and Klebsiella pneumoniae from 2003 to 2009 is a rapid increase in the first and then a slow decline.Conclusion Clinicians Long-term heavily use of antibiotics,making the drug resistant rate of gram-negative bacteria be increased year by year,but drug resistance has been controlled certainly in recent years due to strengthening the hospital infection surveillance,and vigorously promoting the rational use of antimicrobial agents.
