2型糖尿病合并脑梗死患者血清超敏C反应蛋白及同型半胱氨酸与颈动脉粥样硬化的关系

摘    要:目的研究2型糖尿病合并脑梗死(DMCI)患者超敏C反应蛋白(hs-CRP)及同型半胱氨酸(Hcy)与颈动脉粥样硬化的关系。方法选择2009年1月—2011年5月在我院神经内科住院的102例首次发病的急性脑梗死患者,其中DMCI患者44例,非糖尿病脑梗死(CI)患者58例。所有患者行颈动脉彩色多普勒超声检查,观察颈动脉有无斑块及斑块的稳定性,并检测hs-CRP和Hcy水平。结果 DMCI组hs-CRP和Hcy水平与CI组比较,差异均有统计学意义(P<0.05)。DMCI组颈动脉粥样硬化的发生率(86.4%)与CI组(69.0%)比较,差异有统计学意义(P<0.05)。DMCI患者中不稳定性斑块组和稳定性斑块组hs-CRP和Hcy水平与无斑块组比较,差异有统计学意义(P<0.05),不稳定性斑块组hs-CRP与稳定性斑块组比较,差异亦有统计学意义(P<0.05)。结论DMCI患者颈动脉粥样硬化的发生率较高,可能与hs-CRP和Hcy升高相关。

Abstract:Objective To explore the correlation of serum high-sensitivity C-reactive protein(Hs-CRP) and homocysteine(Hcy) with carotid stenosis in patients with acute cerebral infarction with type 2 diabetes mellitus(DMCI).Methods Totally 102 acute cerebral infarction patients,including 44 DMCI patients(DMCI group) and 58 patients with non-diabetic acute cerebral infarction(CI group) were enrolled in this study.The serum hs-CRP and Hcy were measured.Carotid artery examination including the inter-media thickness and carotid atheromatous plaque were performed for all patients by the Doppler ultrasonography.Results The serum hs-CRP and homocysteine levels in DMCI group and in CI group showed significant difference(P<0.05).The incidence of carotid artery atherosclerosis was 86.4% in DMCI group and 69.0% in CI group(P<0.05).Inside the DMCI group,the serum levels of hs-CRP and Hcy also showed significant differences between patients with unstable/stable plaque and those without plaque;in addition,the serum hs-CRP level in patients with unstable plaque also differed from those with stable plaque(P<0.05).Conclusion DMCI patients are more susceptible to carotid artery atheroscleros,which may be related with the increased serum hs-CRP and Hcy levels.
