Application of Health Management Platform in Family Doctor Service System

The service center has adopted a new type of health management platform supported by information technology and based on the updating of residents′ health records,family doctors and other medical data.The service center can provide health service management,infectious disease management and warning,chronic disease management,management of disease prevention,disease monitoring and referral guidance for community people,enabling a better development of Shanghai family doctor service and the establishment of a more human-oriented,individualized and accessible management mode of health service responsibility system for community people including building staff,students and community residents.
