胸痛并慢性肾功能不全患者的冠状动脉造影分析

目的探讨胸痛伴慢性肾功能不全(CRI)患者的临床特征,并对这类患者冠状动脉造影结果进行分析。方法选取2009年10月—2011年6月因胸痛行冠状动脉造影患者384例为研究对象,根据其冠状动脉造影前血清肌酐水平,分为胸痛伴CRI组(n=176例)和单纯胸痛组(n=208例),对比分析两组患者的临床特征及冠状动脉造影资料。结果胸痛伴CRI患者的年龄、合并高血压及2型糖尿病患者比例、超敏C反应蛋白、内生肌酐清除率及B型脑利钠肽水平与单纯胸痛组比较,差异均有统计学意义(P<0.05)。胸痛合并CRI患者冠心病发生率明显高于单纯胸痛组(P<0.01),在冠状动脉造影结果中,胸痛伴CRI患者多支冠状动脉病变、弥漫性病变发生率与单纯胸痛比较,差异有统计学意义(P<0.01)。结论胸痛患者合并CRI时冠心病发生率高,临床特征更趋复杂,且复杂冠状动脉病变发生率较单纯胸痛患者显著增高。

Coronary Angiography in Chest Pains Patients with Chronic Renal Insufficiency

Objective To study the clinical features of chest pains(CP) complicated by chronic renal insufficiency(CRI) and analyze the results of coronary angiography(CA).Methods A total of 384 patients undergoing CA due to CP were divided,according to serum creatinine level before CA,into groups CP+CRI(n=176) and CP(n=208).The clinical features and CA data were compared between 2 groups.Results There was significant difference in age,number of patients with hypertension and T2DM between 2 groups(P<0.05).Coronary disease(CD) incidence was significantly higher in CP + CRI group than in CP group(P<0.01),and in CA results,the incidence of multivessel and diffuse diseases was higher(P<0.01).Conclusions CP+CRI patients have higher CD incidence with complex clinical features.
