奥氮平治疗老年性痴呆患者精神行为症状的疗效观察

目的探讨奥氮平治疗老年性痴呆的精神行为症状的临床疗效。方法对2011年1—10月在本院住院或门诊就诊的74例老年性痴呆伴发精神行为症状患者采用奥氮平进行治疗,观察8周。于治疗前及治疗1、2、4、8周末分别采用蒙特利尔认知评估量表(MoCA)及简明精神量表(BPRS)评定疗效。结果治疗8周后MoCA总分为(20.196±4.537)分,治疗前为(17.332±5.241)分,治疗前后比较差异有统计学意义(t=1.745,P<0.05)。治疗后1、2、4、8周BPRS评分总分值和各因子分值与治疗前比较,差异均有统计学意义(P<0.05),且治疗后1、2、4、8周间BPRS评分总分值和各因子分值比较,差异亦均有统计学意义(P<0.05)。不良反应包括嗜睡7例,乏力3例,头晕3例,头痛1例,行走不稳1例,未发生心脏功能障碍。结论奥氮平治疗老年性痴呆伴发精神行为症状疗效肯定,安全性高。

Clinical Efficacy of Olanzapine in the Treatment of Behavioral and Psychological Symptoms of Dementia in Senile Patients

Objective To investigate the efficacy of Olanzapine in the treatment of behavioral and psychological symptoms of dementia(BPSD) in senile patients.Methods 74 senile patients with BPSD admitted to our hospital or at clinic from January 2011 to October 2011 were treated with Olanzapine and all the patients were observed for eight weeks.Brief psychiatric rating scale(BPRS) and Montreal cognitive assessment(MoCA) were used to assess the efficacy before the treatment and at the first week,the second week,the fourth weeks and the eighth week of the treatment.Results After eight weeks treatment,the total score of MoCA was(20.196±4.537),and the score was(17.332±5.241) before the treatment.The difference was statistically significant(t=1.745,P<0.05).The total BPRS score and each factor score at the first week,the second week,the fourth week and the eighth week of treatment all showed statistically significant differences compared with those before the treatment(P<0.05).The total BPRS score at the first week,the second week,the fourth week and the eighth week of treatment all showed statistically significant differences compared with each factor score(P<0.05).Adverse reactions were somnolence in seven cases,feebleness in three cases,dizziness in three cases,headache in one case and unsteady walking in one case.No one had cardiac dysfunction.Conclusion The clinical efficacy of Olanzapine in the treatment of BPSD in senile patients is positive with good safety.
