重型颅脑损伤并肺部感染(附53例报告)

目的讨论重型颅脑损伤并肺部感染的相关因素及防治。方法回顾我院2004年1月至2009年1月：住院257例重型颅脑损伤患者，合并肺部感染53例临床资料。结果重型颅脑损伤合并肺部感染率20．6％，使用呼吸祝感染率53．1％，肺部感染死亡率与植物生率38．8％。非肺部感染死亡率与植物生存率16．7％有显著差异。结论肺部感染与颅脑损伤程度，年龄（老年人）胸部外伤，治疗措施有关，肺部感染直接影响临床预后，临床上救治重型颅脑损伤原发病时应随时警惕肺部感染的出现并作出及时处理，以取得满意疗效。

Severe Craniocerebral Injury with Pulmonary Infection(Report of 53 Cases)

Objective To discuss the correlation factors, prevention and treatment for severe craniocerebral injury ,vith pulmonary infection.Methods The medical files of 53 cases of severe craniocerebral injury with pulmonary infection were retrospectively collected and analyzed from 257 patients who were admitted to our hospital from January, 2004 to January, 2009.Results Cases with pulmonary infection accounted for 20.6% in patients of severe craniocerebral injury, the ratio was higher at 53.1% for that with assisted ventilation. The mortality and vegetative living accounted for 38.8% for patients with pulmonary infection, compared with that of 16.7% for patients with non-pulmonary infection. The difference was statistically significant.Conclusions Pulmonary infection was correlated with the severity of craniocerebral injury, the age, thoracic injury and therapeutic measures, which had a direct impact to clinical prognosis.To achieve satisfactory therapeutic effect, pulmonary infection should be taken into alert and handled in time in the treatment of severe craniocerebral injury.
