2004年广州市4例散发严重急性呼吸综合征患者实验室检查特点

目的探讨散发严重急性呼吸综合征(SAILS)患者实验室检查的特点。方法分析2004年确诊4例散发SAILS患者的实验室检查结果。结果白细胞及淋巴细胞呈不同程度降低，下降时间较流行时提早，下降程度较小。2例外周血T淋巴细胞亚群计数轻微降低。3例丙氨酸氨基转移酶(ALT)和天冬氨酸氨基转移酶(AST)轻度升高。肾功能、血糖、血脂及心肌酶无异常。血氧饱和度均无降低。所有病人血清SAILS-IgM／G抗体早在发病第6天由阴转阳，滴度在短期内呈4倍以上升高，中和抗体实验阳性，1例咽拭子中检测出SAILS-CoV RNA。结论2004年中国4例散发病例，临床表现较轻，病程短，无并发症，归因于此次致病的SAILS-CoV毒力小，抗体出现较早，病毒迅速被清除。

The laboratory characteristic of 4 patients with sporadic severe acute respiratory syndrome

Objective To explore the laboratory characteristics of patients with sporadic severe acute respiratory syndrome (SARS) Method Analyse the laboratory findings of the 4 cases of SARS occurring in 2004 Results Leucocytopenia and lymphocytopenia were seen in all the patients Two patients had slightly decreased peripheral blood T lymphocyte count ALT and AST levels increased slightly in 3 (75%) patients No desaturation of oxygen was seen in all the patients Both SARS IgM and IgG sero conversion occurred earlier in all the patients with the titer increasing for more than 4 folds shortly Neutralization test was positive in all the patients SARS CoV RNA was detected by PCR in pharyngo swabs only in one patient Conclusions The four sporadic SARS patients in 2004 have milder manifestation, shorter course of disease and no complications than that of before This attributes to smaller virulence of the SARS CoV The antibody appears earlier in these patients The SARS CoV was eliminated rapidly
