升主动脉瘤的外科治疗

目的探讨升主动脉瘤治疗经验。方法Marfan’s综合征12例，其中6例合并二尖瓣关闭不全。升主动脉夹层动脉瘤伴主动脉瓣关闭不全6例，升主动脉瘤合并主动脉瓣、二尖瓣关闭不全2例，单纯升主动脉瘤5例，行Bentall手术12例，Cabrol手术3例，Wheat手术5例；单纯升主动脉替换术5例。结果全组无手术死亡。结论Bentall手术是外科治疗升主动脉瘤的主要术式。早期诊断、早期手术是改善升主动脉瘤患者疗效的关键。

Surgical treatment of ascending aortic aneurysm

Objective To investigate the surgical experience in treatment of ascending aortic aneurysm from May 1999 to December 2006.Methods In the series,12 cases were diagnosed as Marfan's syndrome,among them,6 cases were accompanied with mitral insufficiency,6 cases had ascending aortic aneurysm with aortic valve insufficiency.Two patients were accompanied with mitral and aortic valve insufficiency.Six cases were subjected to resection of ascending aneurysm with mitral valve plasty,2 to resection of ascending aneurysm with graft and aortic valve and mitral replacement.Bentall technique was performed on 12 cases and Cabrol technique on 3 case.Graft replacement of ascending aortic aneurysm alone was done in 5 cases.Results There was no operative death in the series.Conclusion Bentall operation is a standard method for treating ascending aortic aneurysm.Earlier diagnosis and prompt operation are essential to improve surgical results of patients.
