改良外剥内扎术治疗混合痔疗效观察

目的为提高混合痔的治疗效果 ,保护肛门肛管功能及肛门正常结构 ,降低术后并发症和后遗症的发生率。方法按国内混合痔暂行诊治标准及排除标准[1] ,纳入病例 14 8例 ,采用简单随机法分为观察组 (n =74 )和对照组 (n =74 )。观察组采用“改良术式———外切内注内扎缝合术”术式 ,对照组用“外剥内扎”术式 ,术后均抗炎、止血、对症治疗。结果观察组 74例 ,一次性治愈 72例 ,占 97 30 % ,2例术后水肿经二次手术治疗痊愈 ,总有效率 10 0 % ;对照组 74例 ,一次性治愈 6 3例 (占 85 14 % ) ,行二次手术治愈者 11例 (占 14 86 % ) (其中包括 :一次手术后水肿形成血栓者 4例、桥形愈合 3例、切口感染 4例 )。结论两组数据对比并经统计学处理 ,p <0 .0 5 ,观察组疗效明显优于对照组 ,肛门功能及结构受损少 ,术后并发症后遗症少 ,疗程短。

The Treatment of Mixed Hemorrhoids with Reformative Operation by Using Incision of External Hemorrhoids plus Suture and Injection of Internal Hemorrhoids

Objective To improve care rate of mixed hemorrhoids and protect the functions of anal,meantime,reduce the postoperative complication, such as pain, bleeding, retention of urine,narrowing of anal skin,etc.Methods We designed the operation by using Incision of external hemorrhoids plus suture and injection of interrral hemorrhoids.148 cases consecutive patients with mixed hemorrhoids were divided into two groups at simple ranodm: Study group (n=74) patients were underwent the operation by using Incision of external hemorrhoids plus selective suture and injection of internal hemorrhoids;Control group (n= 74) patients were underwent the classical operation by external excision and internal separation with ligational. Result Study group had 73 cases (97.30%) one care rate,two patient (2.70%) had secondary operation ;The control group had 63 cases (85.14 %) one care rate and 11(14.86%) cases patient had secondary operation. Conclusion Our study shows that the study operation has move advantages,less surgical injury, and shorter treatment course, raising cure rate,and in can reduce the postoperative complication and protace the anal functions..
