200例肺癌的诊断及手术治疗体会

本文报告我院1981～1986年200例肺癌外科手术治疗的经验。术前误诊为纵隔肿瘤、肺结核瘤、肺脓肿、肺炎住假瘤等40例,误诊率为20%。行肺叶切除123例;其中2例术后复发,余肺再次切除。复合切除50例;袖状肺叶切除17例,术后无瘘和吻合口狭窄。我们认为熟练的吻合技术是手术成败的关键。全肺切除10例。对14例肺门、纵隔有淋巴结转移的中央型肺癌,术前进行了半量放疗(3000rad),术中均能成功的切除。术后29例轻型并发症全部治愈,1.例术中死于心脏骤停。

The surgical treatment of 200 cases of lung cancer

In this artick, 200 cases of lung cancer opereted on in our hospital from 1981 1986 were reported.40 pts out of the 200 were misciagnosed before the operationsas mediastinal tumour, tumouloma, Lung abscess and pueumonitic pseudotumor.Theof misdiagnisis was 20%, Lobectomy was operated on 123 patiants and 2 of themwere operated on twece, owing to the postperative local recurrence. Compound rasesection was pertormed on 50 patients,total pneumonectomy on 10pta,and sleeve lobectomy on 17 patients in whom no bronchpleural fistulas and stenosis ware tound.we consider that skilful technique for bronchial anastomosis is the pre-requisitefor success of operation. 14paticnts with ccentral typs heviag lymphatic metastasisof hilar and madiastinum accepted preoperative half-dose radiationtherapy, and suc-cessful resections were obtained. 29pts with light complications after operation fu-lly recovered in a short time and there was only one death owing to cardiac su-dden stop during ihe operation.
