不稳定型心绞痛降纤酶治疗后纤溶特性的变化

摘    要:目的:探讨不稳定型心绞痛(UAP)经降纤酶治疗后血纤溶特性的变化。方法:选择符合UAP诊断标准的80例病人,男性47例,女性33例,年龄(60±s 10)a,39—78 a,随机分为治疗组及对照组,2组均给予 UAP常规治疗,治疗组在此基础上用降纤酶10 U,iv,gtt,qd,共5 d,治疗前后空腹抽血检查血纤溶及凝血指标。结果:治疗后治疗组血纤维蛋白原(Fib)由(5.7±1.3)g·L-1降低为(4.6±1.0)g·L-1、纤溶酶原激活物抑制因子(PAI-1)由(79±20)μg·L-1降低为(68±11)μg·L-1,组织型纤溶酶原激活物(t—PA)由(52±24)μg· L-1增加为(65±10)μg·L-1,对照组仅t—PA有所增加,而Fib和PAI-1的变化差异无显著意义。2组D-二聚体、血小板α颗粒膜蛋白、血管性假血友病因子变化均无显著差异。结论:降纤酶能增加不稳定型心绞痛的纤溶活性。

Abstract:AIM:To study the change of fibrinolytic systems in patients with unstable angina pectoris (UAP) after defibrase (DF). METHODS: Eighty patients of UAP were randomly divided into two groups, DF group(41 patients) and control group (39 patients). Patients in both groups were taken the basal of conventional oral remedy of UAP ,with additional treatment for DF group patient of defibrase under the dose of 10 U,iv,gtt, qd × 5 d.The fasting serum plasminogen and coagulase were tested before and after the treatment. RESULTS: After the treatment DF group showed significant reduction of fibrinogen, plasminogen activator inhibitor-1 (PAI-1) but the tissue plasminogen activator(t-PA) increased in comparing with the control group, there were statistical significance. The differences of D-dimer,alpha-granule membrane protein of platelet,and von Willebrand factor between the two groups did not reach statistical significance after the therapy. CONCLUSION: DF has obvious effect on UAP in improving the fibrinolytic system characteristics.
