老年人活动性义齿所致食管异物的诊治

目的总结食管义齿异物病例的临床特点及诊治经验。方法对1999年1月-2008年12月所收治的8例食管义齿异物病例进行回顾分析,就其病例特点及诊治经验进行总结。结果共收治食管义齿异物8例,男、女各4例,平均年龄73.3(65～86)岁;自发病到接受诊疗的时间从2 h到2个月不等;义齿在食管内均已发生嵌顿,除1例发生于胸段食管外,余均发生于颈段食管。采用局麻硬食管镜手术取出异物4例,全麻硬食管镜手术取出异物2例,全麻下颈侧入路食管切开手术取出异物2例。多数义齿异物带有金属卡环及数量不等的尖锐齿钩。所有病例均未发生严重的并发症,平均住院日为3.5(1～8)d。结论食管义齿异物常见于老年人,易在食管内形成嵌顿,可选择全麻下硬食管镜异物取出术,必要时亦可采取经颈或经胸食管切开异物取出手术,应加强对佩带义齿老年人的宣教工作以减少食管异物发生。

Managements of esophageal foreign body dentures in elderly
