支气管内超声引导针吸活检术在纵隔和肺门肿大淋巴结的应用价值

目的探讨支气管内超声引导针吸活检术(EBUS-TBNA)在胸部疾病诊断中的应用价值。方法分析2012年2～6月32例患者经EBUS-TBNA检查肺部增强CT提示纵隔和肺门肿大淋巴结的临床资料。其中,男18例,女14例;平均年龄49.3岁。3例EBUS-TBNA检查仍未能明确诊断者,进一步接受外科手术活检或临床观察。结果经EBUS-TBNA检查后29例获得明确诊断,其中,恶性病变20例,良性病变9例,确诊率为90.625%。EBUS-TBNA在纵隔和肺门肿大淋巴结诊断和鉴别诊断方面的敏感性、特异性和准确性分别为93.1%、100%和为94.7%。所有受检者耐受良好,无相关并发症发生。结论对于纵隔和肺门肿大淋巴结病变,EBUS-TBNA是一种安全、有效的诊断方法。

Application value of endobronchial ultrasound-guided transbronchial needle aspiration in diagnostic value of mediastinal and hilar lymphadenopathy

Objective To explore the application value of endobronchial ultrasound-guided needle aspiration biopsy(EBUS-TBNA)in the diagnosis of chest diseases.Methods Reviewed the data of 32 patients examined with EBUS-TBNA in our institution from February to June 2012,and all of them the lung enhanced CT prompted mediastinal and hilar lymph nodes.Among the patients,there were 18 males and 14 females,an average age of 49.3 years old.Three cases of EBUS-TBNA examination had yet to confirm the diagnosis,further to surgical biopsy or clinical observation.Results The diagnosis of 29 cases had been confirmed by EBUS-TBNA examination.Among them,20 cases of malignant lesions,benign lesions in 9 cases,and the diagnosis rate was 94.7%.The sensitivity,specificity and accuracy of EBUS-TBNA in the diagnosis and differential diagnosis of mediastinal and hilar lymph nodes was 93.1%,100%and 94.7%.All patients showed well tolerance,and no related complications.Conclusion For lesions of mediastinal and hilar lymph nodes,EBUS-TBNA is a safe and effective diagnostic method.
