小剂量血浆置换联合大剂量激素冲击治疗重症肌无力的疗效观察

目的探讨小剂量血浆置换联合大剂量激素冲击治疗重症肌无力的疗效。方法选取2008年3月～2012年1月本院及两家联合医院收治的52例重症肌无力患者作为研究对象,将上述患者随机分为两组,所有患者均接受吡啶斯的明治疗,观察组(26例)在此基础上联合接受小剂量血浆置换与大剂量激素冲击治疗,对照组(26例)在此基础上联合接受大剂量激素冲击治疗。结果观察组治疗前后临床绝对评分的降低幅度显著大于对照组,两组比较差异有统计学意义(P<0.05)。观察组治疗起效时间显著短于对照组,两组比较差异有统计学意义(P<0.05)。所有患者均有不同程度的库欣反应,两组患者库欣反应发生率比较,差异无统计学意义(P>0.05)。结论小剂量血浆置换联合大剂量激素冲击治疗重症肌无力具有起效快、疗效良好、不良反应无显著增多等优点。

Efficacy observation of low-dose plasma exchange combined with high-dose hormone impact in the treatment of myasthenia gravis

Objective To explore the therapeutic efficacy of low-dose plasma exchange combined with high-dose hormone impact in treatment of myasthenia gravis.Methods Fifty two cases of myasthenia gravis who were admitted to our hospital and two united hospital from March 2008 to January 2012 were divided into two groups randomly.The observation group(n =26)was given low-dose plasma exchange combined with high-dose hormone impact based on pyridostigmine,while control group(n=26)was given high-dose hormone impact based on pyridostigmine.Results The reduced extent of clinical absolute scores of observation group between before and after treatment was significantly lager than that of control group(P<0.05).The onset time of observation group was significantly shorter than that of control group(P<0.05).All patients had varying degrees of Cushing reaction,and there was no significant difference in the incidence rate of Cushing reaction between two groups(P>0.05).Conclusion Low-dose plasma exchange combined with high-dose hormone impact in the treatment of myasthenia gravis shows rapid onset,good efficacy,with no increase in adverse reactions.
