生长抑素对颈淋巴结清扫术后乳糜漏的疗效观察

摘    要:目的观察应用生长抑素治疗颈淋巴结清扫术后乳糜漏的临床疗效。方法回顾分析2010年1月至2012年12月应用生长抑素联合局部加压、负压引流、低脂饮食控制治疗的11例颈部淋巴结清扫术后乳糜漏患者临床资料。结果11例中10例患者在治疗2—3天后漏液明显减少,引流量少于10mL拔除引流管。1例效果仍不佳者予手术治疗后痊愈。结论生长抑素联合局部加压、负压引流、低脂饮食治疗颈淋巴结清扫术后乳糜漏效果明显,可缩短患者住院时间,降低再手术率。

The obseration of treatment in use of somatostatin in chylous leakage after neck dissection

Abstract:Objective To observe the clinical curative effect on the treatment of somatostatin on chylous effusion after the operation of lymphonodi eervicales. Methods Of all the 21 cases of chylous fistula after radical neck dissection from January 2010 to December 2012 of head and neck department in Jiangsu cancer hospital, 21 cases were treated with conservative method (partial pressurization, negative pressure drainage, and low fat diet), 10 cases were healed and 11 cases were retreated with somatostatin. Meanwhile, press it partially, do the negative pressure drainage, and control the low fat diet. Then compare these treatments separately. Results Effect of somatostatin and other methods on chylous effusion after the operation of lymphonodi cervicales is better than that of not use somatostatin. Fluid volume rapidly reduced after these treatments, when drainage volume reduced to under 10 ml, we can remove drainage tube. 10 cases were cured after conservative treatments and 1 case was cured after reoperation. Conclusions Somatostatin and other methods on chylous effusion after the operation of lymphonodi cervicales, could raise the closing rate of eyhle fistula, shorten the median length of hospital stay, which could be an effective therapeutic way of chylous effusion after the operation of lymphonodi cervicales.
