妊娠肝内胆汁淤积症患者胎盘血管内皮生长因子的表达及其意义

摘    要:目的 观察血管内皮生长因子 (VEGF)在妊娠肝内胆汁淤积症 (ICP)胎盘中的表达。方法 采用免疫组织化学方法 ,检测 2 5例正常妊娠胎盘 (对照组 )和 2 5例ICP胎盘 (ICP组 )中VEGF的水平。结果 VEGF在正常妊娠胎盘和ICP胎盘中分布基本一致 ,分布在胎盘滋养叶细胞、血管及绒毛间质。在ICP组VEGF为轻度表达占 72 % ,中度表达占 2 0 % ,重度表达占 8% ;而在对照组VEGF轻度表达占 2 4 % ,中度表达占 4 4 % ,重度表达占 32 %。ICP组中VEGF表达明显低于对照组 (P <0 0 1)。结论 VEGF在胎盘中主要由合体滋养细胞分泌 ,ICP胎盘中VEGF的减少可能与胎盘血管生成减少及胎盘滋养叶细胞侵入异常有关 ,在ICP发病中占有一定的地位。

Abstract:Objective To investigate the expression of vascular endothelial growth factor(VEGF)in placenta from patients with intrahepatic cholestasis of pregnancy(ICP).Methods The expression of VEGF in 25 placentas from normal pregnancy(normal group)and 25 placentas from ICP(ICP group)was detected by immunohistochemistry.Results The distribution of VEGF in placenta was found in villous trophoblast,villous blood vessels and villous stroma in both groups.But the levels of expression were significantly different:light expressed in ICP group about 72%,moderate expression 20% and strong expression 8%.In normal placenta light expression accounted for only 24%, moderate 44%, and strong 32%.The level of VEGF was significantly lower in ICP group than that in normal group(P<0.01).Conclusion These results suggest that VEGF is secreted mainly by syncytiotrophoblast in human placenta.The decrease of VEGF in placenta from ICP may be related to inadequate blood vessel formation and the abnormal invasions of trophoblast and may play a role in the cause of ICP.
