洛赛克治疗上消化道出血40例临床疗效分析

以H＋－K＋－ATP酶（质子泵）抑制剂洛赛克（losec）静脉注射治疗上消化道出血患者40例，有效率为92．5％，与对照组有效率70％相比较，差异有显著性（P＜0．05）。洛赛克对上消化道出血的止血效果显著，止血作用迅速而持久，但加大剂量并不能使疗效呈比例增强。推荐首日剂量80mg，以后每日40mg较为合适。病因不同，其疗效也不一致。疗效高低依次为急性胃粘膜病变，十二指肠溃疡，糜烂型胃炎，胃溃疡；而对胃癌、肝硬化所致食管胃底静脉曲张破裂出血疗效较差，洛赛克不宜作为首选药物。

ANALYSIS OF 40 CASES OF HEMORRHAGE OF UPPER DIGESTIVE TRACT BY INTRAVENOUS LOSEC INJECTION

Forty cases of acute hemorrhage of upper digestive tract were treated by intravenous injection with losec (Omeprazole), a inhibitor of H+-K+-ATP enzyme or proton pump. The effective rate of rapid stoppage of bleeding was 92. 5%, higher than that of the controlled (70% ) (P<0. 05). The results of this observation indicate that by intravenous injection with losec is effective in treating hemorrhage of upper digestive tract but the effectiveness is not proportional to the drug dosage. The initial dose of losec is 80mg in the first day, then followed by daily dose of 40mg. Our results also confirm that the therapeutic effect of losec is dependent on the pathology. It is most effecti ve of losec in acute gastric mucosa erosion, duodenal ulcer, erosive gastritis and gastric ulcer in sequence, losec,howerer,is rarely recommended in the treatment of hemorrhage caused by gastric cancer or ruptured varices of lower esophagus or gastric fundus due to portal hypertension.
