眶上缘骨折临床分析

目的 探讨眶上缘骨折的临床表现和手术治疗方法.方法 回顾分析2007～2014年在我科治疗的7例眶上缘骨折患者的临床表现、辅助检查和手术治疗结果.结果 7例患者中主诉视物不适4例,其中3例行黑斯屏检查提示存在眼肌功能轻度障碍.明显复视3例.CT扫描显示粉碎骨折4例,非粉碎骨折3例,均显示骨折片压迫上直肌.7例患者行手术治疗,经原伤口入路行碎骨片清除术和/或骨折复位内固定术.所有患者术后症状缓解.结论 骨折片压迫上直肌造成眼球运动障碍是眶上缘骨折的主要临床表现,解除上直肌受压和恢复眶上缘结构是其手术要点.

A clinical study of supraorbital rim fracture

Objective To investigate the clinical manifestation and surgical approach of supraorbital rim fractures.Methods Seven supraorbital rim fracture patients from 2007 to 2014 were retrospectively reviewed.All patients received CT scan and three cases received HESS charting examinations.Ebonation and/or fractured bone reduction and rigid fixation were performed on all seven patients.The patients were followed up for six months after surgery.Results Diplopia was the chief complain in three patients and the other four patients complained eye discomfort.CT scan showed supraorbital rim fractures in seven cases including four case of communicated fractures and superior rectus compression.Three cases of HESS charting examination indicated extraocular muscle movement restriction.The symptoms improved after surgery in all patients.Conclusion Eye movement restriction due to superior rectus compression is the main symptom of supraorbital rim fractures.The key point of the surgery is superior rectus decompression and orbital rim reconstruction.
