血糖水平在判断合并应激性高血糖的ICU患者预后中的应用价值

目的探讨不同血糖水平对ICU患者预后判断的临床意义。方法将150例合并应激性高血糖的ICU患者随机分为3组，每组50例，其中A组血糖≤6．0mmol／L，B组血糖为6．1～10．0mmol／L，C组血糖≥10．0mmol／L，对比和分析3组的应用效果。结果A组的住院时间、呼吸机通气时间短于B组，低血糖发生率和死亡率高于B组，差异有统计学意义（P〈0．05）；A组的住院时间、呼吸机通气时间短于C组，多器官功能衰竭发生率和死亡率低于C组，低血糖发生率高于C组，差异有统计学意义（P〈0．05）。结论血糖水平对ICU患者的预后会产生一定影响，对于合并应激性高血糖的ICU患者而言，血糖水平应控制为6．1。10．0mmol／L为宜

Application value of glucose level in determining the prognosis of ICU pa- tient combined with stress hyperglycemia

Objective To investigate the clinical significance of the different glucose level in ICU patients prognosis. Methods 150 cases of critically ill patients combined with stress hyperglycemia were randomly assigned to three groups,50 cases of each group.Glucose level in group A was 〈6.0 mmol/L,glucose level in group B was 6.1 to 10.0 mmol/L,glucose level in group C was I〉 10.0 mmol/L.The application effect of three groups was compared and analyzed. Results The hospitalization time,ventilator time in group A was shorter than that in group B respectively,the incidence rate of hypoglycemia and mortality in group A was higher than that in group B respectively,with statistical difference（P〈 0.05）.The hospitalization time,ventilator time in group A was shorter than that in group C respectively,the incidence rate of multi-organ failure and mortality in group A was lower than that in group C respectively,the incidence rate of hypo- glycemia in group A was higher than that in group C,with statistical difference （P〈O.05）. Conclusion The glucose level have an impact on the prognosis of ICU patients,the most appropriate glucose level should be between 6.1 to 10.0 mmol/L in ICU patient with stress hyperglycemia.
