不同时间窗早期干预新生儿缺氧缺血性脑病对其预后的影响

目的分析不同时间窗早期干预对新生儿缺氧缺血性脑病（HIE）预后的影响。方法回顾性分析本院收治的105例中重度HIE患儿,根据就诊时间分为3组：出生后6 h内为Ⅰ组、6~24 h为Ⅱ组、〉24 h为Ⅲ组。每组患者均给予相同的综合对症支持治疗。于患儿出院后3、6、12个月龄来院复诊时评价其神经系统发育程度。结果治疗后,Ⅰ、Ⅱ、Ⅲ组患儿的心理发育指数（MDI）、运动发育指数（PDI）在3个时间点（3、6、12个月龄）的变化差异有统计学意义（P〈0.05）,Ⅰ组患儿的MDI、PDI水平高于Ⅱ组、Ⅲ组,差异有统计学意义（P〈0.05）,但Ⅱ组与Ⅲ组差异无统计学意义（P〉0.05）。结论出生后6 h内可能是HIE患儿的最佳治疗时间窗。

The influence of prognosis for intervention by different time window on the neonatal hypoxic ischemic encephalopathy

Objective To analyze the influence of prognosis for intervention by different time window on the neonatal hypoxic isehemic eneephalopathy （HIE）.Methods 105 cases of patients with severe HIE who were treated in our hospi- tal were retrospectively analyzed,and were divided into group [ （within 6 hours after birth）,group II（6-24 hours after birth）,group III （〉24 hours after birth） according to treatment time of children.Each group of patients were given the same comprehensive symptomatic treatment.After discharge,the nervous system development of patients who were in 3,6,12 months was evaluated.Results After treatment,the mental development index （MDI）,psychomotor development in- dex （PDI） level at 3 points time （3,6,12 months） changes of 3 groups had significant defferenee （P〈0.05）.The MDI,PDI level of group I was higher than that of group II and group III,with significant difference （P〈0.05）,but the MDI,PDI level of group II and group III had no significant differences （P〉0.05）.Conclusion Within 6 hours after birth may be the best therapeutic time window of HIE children.
