开颅夹闭和血管栓塞治疗脑动脉瘤的临床效果与安全性比较

目的比较开颅夹闭和血管栓塞治疗脑动脉瘤的临床效果与安全性。方法选取2010年2月~2013年3月于本院进行治疗的76例脑动脉瘤患者为研究对象,将其随机分为A组（开颅夹闭治疗组）38例和B组（血管栓塞组）38例,比较两组中不同Hunt-Hess分级者的格拉斯哥预后评分（GOS）结果及并发症发生率、住院时间、复发率。结果两组患者不同Hunt-Hess分级者GOS 5分者的比例及复发率差异无统计学意义（P〉0.05）;B组的并发症发生率低于A组,住院时间短于A组,差异有统计学意义（P〈0.05）。结论开颅夹闭和血管栓塞治疗脑动脉瘤的临床效果无明显差异,血管栓塞的安全性相对更高,患者恢复更快。

Comparison on the clinical effect and safety of craniotomy clipping and vascular embolization in treatment of cerebral aneurysm

Objective To observe and compare the clinical effect and safety of craniotomy clipping and vascular em- bolization in the treatment of cerebral aneurysm.Methods 76 cases of patients with cerebral aneurysm in our hospital from February 2010 to March 2013 were selected,and were randomly divided into group A （craniotomy clipping treat- ment group） 38 cases and group B （vascular embolization group） 38 cases,then the Glasgow outcome scale （GOS） of different Hunt-Hess stages and complications rate,hospitalization time,relapse rate of two groups was compared.Results The different Hunt-Hess stages GOS 5 scores and relapse rate of two groups had no significant difference （P〉0.05）; while the complications rate of group B was lower than that of group A,hospitalization time was shorter than that of group A, the difference was significant （P〈O.OS）.Conclusion The clinical effect of craniotomy clipping and vascular em- bolization in treatment of cerebral aneurysm is not obviously different, the safety of vascular embolization is respectively higher,the recovery of patients is faster.
