摘    要:目的:探讨拱形Medpor假体在矫正外伤性骨性鼻畸形中的作用。方法:先行鼻侧截骨和突出骨畸形修整后,植入拱形Medpor用于重建鼻根和鼻背中线挺拔和平整的外形。37个病例,平均年龄27.4岁,35例男性,2例女性,伤后平均13个月接受手术。结果:术后随访7.6个月,病人鼻畸形100%得以改善,病人满意率92%(34/37),无感染和排斥反应出现。结论:拱形Medpor假体植入对于纠正外伤性骨性鼻畸形有良好作用,可以作为首选植入材料,远期的效果及与周围骨性结构的变化还有待于观察。

Abstract:Objective Traumatic nasal deformity present in many type of malformation, mostly depending on which component of were involved. Because of fragment of nose bone and adjarant bone, to restore these crushed small bone is difficult. This article present our experiences in the correction of traumatic nasal deformity of skeletal structure by inserting arch shape of the Medpor (porous polyethylene). Methods From April 2003 to June 2012, 37 patients, 27.4 years old on average at the surgery, including 35 males and 2 females, underwent rhinoplasty with mainly correcting skeletal deformity. The interval from injury to the rhinoplasty procedure was 13 months on average.The surgical procedures is consisted of the lateral nasal osteotomies and carving irregularity of protrude bone followed by insertion of arch shape of Medpor implant. Results The patients were followed up for average period of 7.6 months. The overal aesthetic improvement rate was 100%, and the patient-satisfaction rate was 92%(34/37). Two patients complainted uncompleted correction of base of nasal arch. One's nasal tip was unsatisfactionay. No infection and implant rejection happened. Conclusion The Medpor implant (porous polyethylene) is an effective alternative to autograft or other biomaterial for correction of skeletal nasal deformity for achieving a pleasing appearance of the nose, the Medpor implant with thin arch shape may be considered to be the first choice. The longer follow-up of the patients is expected to understand the long-term effect of combination of the implant and bone nearby.
