鞍结节脑膜瘤显微手术治疗

目的 探讨鞍结节脑膜瘤的手术入路和手术技巧及治疗效果。方法 回顾性分析显微手术治疗的25例鞍结节脑膜瘤。肿瘤大小2．5-6cm，病人均表现为视力减退。均行开颅手术，单侧额下入路14例，翼点入路8例，双额经纵裂入路3例。结果SimpsonⅠ级切除9例，Ⅱ级切除16例，无手术死亡。术后视力改善16例，无变化5例，视力减退4例。结论 应用恰当手术入路及显微神经外科手术技术，大多数鞍结节脑膜瘤可以全切除，并改善病人视力。

Microsurgical treatment of tuberculum meningiomas

Objective This study was to describe the surgical approach and technique of tuberculum sellae meningiomas.Methods Twenty-five patients with tuberculum sellae meningiomas were analyzed retrospectively.The tumor size ranged from 2.5 to 6 cm.All patients presented with symptoms of visual dysfunction.All patients underwent craniotomy.Fourteen cases underwent unilateral subfrontal approach,pterional transsylvian approach in 8 cases, bilateral frontal approach in 3 cases.Results Simpson I grade resection was achieved in 9 cases,Simpson II grade resection in 16 cases.Postoperative visual acuity was improved in 16 cases,unchanged in 5 cases,and worsened in 4 case.Conclusion In the majority of patients with tuberculum sellae meningiomas,total resection may be achieved by using correct approach and microneurosurgical technique with visual acuity improved.
