芬吗通改善反复胚胎移植失败患者子宫内膜血流和提高妊娠率的研究

目的探讨芬吗通在反复移植失败患者中改善子宫内膜血流及提高临床妊娠率的作用。方法选择2011年5月至2012年12月在我中心接受胚胎解冻移植(FET)助孕,且彩色多普勒能量图提示子宫内膜血流不良(无血流信号和仅内膜下可及血流信号)的反复胚胎移植失败的不育症妇女为研究对象,共404例。应用芬吗通治疗1²个疗程,于移植前一日进行子宫内膜血流检查,分析子宫内膜血流改善情况,随访FET结局。结果 404个研究周期中,其中336例进行了移植周期,胚胎种植率26.19%,临床妊娠率44.04%。完成404个研究周期用药前后子宫内膜血流的检测,血流改善283例,改善率70%。用药前,B超示无血流信号、仅内膜下可及血流信号、内膜下及内膜内均可及血流信号的患者所占比例分别为81.4%、18.6%和0%,服用芬吗通后三者的比例分别为25.5%、23.8%和50.7%,内膜下及内膜内均可及血流信号的患者所占比例明显升高,差异有统计学意义(P<0.01)。结论芬吗通可明显改善反复移植失败患者的子宫内膜血流情况,提高患者的胚胎种植率及临床妊娠率。

Femoston improves endometrial blood flow and increases pregnancy rate of patients with repeated implantation failure

Objective： To study the role of Femoston with repeated implantation failure（RIF）. in endometrial blood flow and pregnancy rate in patients Methods： The data of 404 frozen-thawed embryo transfer（FET）cycles in RIF patients with sub- endometrial blood flow or no blood flow detected in our hospital from 2011 to 2012 were retrospectively analyzed. Femoston was used and endometrial blood flow and thickness were measured in the morning before transferring. Results： In 336 FET cycles,the clinical pregnancy rate was 44. 04% and the embryo implantation rate was 26.19%. Before using Femoston, the ratios of undetectable blood flow, detectable sub-endometrial blood flow only, and detectable blood flow in both endometrium and sub-endometrium were 81.4M, 18. 6%,0% respectively. After using Femoston,the ratios were significantly changed to 25.5%,23.8%, 50.7% respectively（P%0.01）. The endometrial blood flow was improved in 70M patients. Conclusions： Femoston could significantly improve the endometrial blood flow and pregnancy rate in RIF patients.
