T_1期高级别尿路上皮癌术后复发的影响因素分析

目的通过对T1期高级别尿路上皮癌患者的术后随访,了解影响这类患者复发的因素。方法回顾性分析78例T1期高级别尿路上皮癌患者,对其年龄、性别、肿瘤多发或单发、肿瘤大小、有无吸烟史等因素进行了χ^2检验及多因素分析。结果78例患者术后复发率34.6%（27/78）,术后2年内复发者19例。吸烟患者46例,均为男性,45.7%（21/46）出现术后复发。χ^2检验示肿瘤大小（P=0.012）和吸烟史（P=0.014）对肿瘤复发的影响有统计学差异,多因素分析示肿瘤数目（P=0.027）、肿瘤大小（P=0.018）和吸烟史（P〈0.001）有统计学差异。结论除了肿瘤数目、肿瘤大小是影响膀胱肿瘤患者预后的重要因素外,有吸烟史的T1期高级别尿路上皮癌患者可能更容易复发。

Analysis the influencing factors of T1 high grade urothelial carcinoma with recurrence after surgery

Objective To investigate the factors influencing the recurrence of patients with T1 high grade urothelial carcinoma by follow-up. Methods 78 cases of T1 high grade urothelial carcinoma patients were analysis retrospectively.Age,gender,tumor of multiple or single factors,tumor size,and smoking history were tested byχ2 test and multivariate analysis. Results 34.6%（27/78）patients was found recurrence after operative.19 cases recurred within 2years after operative.In46 male patients with smoking history,45.7%（21/46）had postoperative recurrence.The tumor size（P=0.012）and smoking（P=0.014）had significant difference byχ2 test.The number of tumor（P=0.027）,tumor size（P=0.018）and smoking（P〈0.001）had significant difference by multivariate analysis. Conclusions The number of tumor,tumor size is an important factor affecting the prognosis of patients with bladder cancer.Smoking history might be one of the important factors influence the recurrence of patients with T1 high grade urothelial carcinoma after operative.
