前列腺癌根治术后tPSA异常的影响因素

目的分析前列腺癌根治术后tPSA异常的影响因素,为前列腺癌患者个体化后续治疗提供依据。方法回顾性分析2004年1月至2013年10月在广西医科大学第一附属医院及附属肿瘤医院行前列腺癌根治术的72例前列腺癌患者的临床资料。单因素分析研究各参数对根治术后1个月tPSA异常的影响,多因素Logistic回归分析影响根治术后tPSA异常的独立因素。结果单因素分析显示年龄、前列腺体积、直肠指检、影像学表现、术前Gleason评分及临床分期对评估术后tPSA值异常无统计学意义（P〉0.05）。新辅助治疗、术前tPSA水平、病理分期、术后Gleason评分、淋巴结转移及切缘情况对评估术后tPSA值异常有统计学意义（P〈0.05）。多因素分析显示术后Gleason评分、淋巴结转移及切缘情况是术后tPSA异常的独立影响因素。结论术后Gleason评分、淋巴结转移、切缘情况是影响前列腺根治术后患者tPSA异常的独立因素。

The influence factors of tPSA failure after radical prostatectomy

Objective To investigate the influencing factors of abnormal tPSA after radical prostatectomy for providing the basis for individualized post-operation therapy of patients. Methods The clinical and pathological data of 72 cases who accepted radical prostatectomy were retrospectively analyzed in the First Affiliated Hospital and Affiliated Tumor Hospital of Guangxi Medical University between January 2004 and October 2013.Univariate analysis was used to estimate the relationship between the parameters and abnormal tPSA（one month after operation testing）.Multivariable logistic regression analysis were used to determine influencing factors for abnormal tPSA. Results The univariate analysis showed that patient′s age,prostate volume,digital rectal examination,imaging findings,preoperative Gleason score and preoperative clinical stage had negative prognostic significance（P〉0.05）.Neoadjuvant treatment,tPSA level,pathological stage,postoperative Gleason score,lymph node metastasis and margin status differences were positive prognostic significant（P〈0.05）.The multifactor analysis revealed that postoperative Gleason score,lymph node metastasis states and margin status were independent predictors of abnormal tPSA. Conclusions Postoperative Gleason score,lymph node metastasis,margin status were independent influencing factors of abnormal tPSA after radical prostatectomy.
