原发性肾病综合征红细胞免疫功能的初步观察

为了解肾病综合征（ＮＳ）患儿的红细胞免疫功能，对３７例原发性ＮＳ患儿红细胞免疫功能进行检测，同时对其中２８例进行了治疗前后动态观察。结果显示：（１）单纯性ＮＳ活动期红细胞Ｃ３ｂ受体花环率（Ｅ－Ｃ３ｂＲＲ）显著降低，血清红细胞免疫粘附抑制率（ＥＩＡＩＲ）显著增高；缓解期Ｅ－Ｃ３ｂＲＲ仍低于对照组；Ｅ－Ｃ３ｂＲＲ降低与Ｔ细胞亚群变化密切相关；（２）肾炎性ＮＳ活动期Ｅ－Ｃ３ｂＲＲ和血清红细胞免疫粘附促进率（ＥＩＡＥＲ）显著降低，红细胞免疫复合物花环率（Ｅ－ＩＣＲ）和ＥＩＡＩＲ明显增高；缓解期皆恢复正常；（３）ＮＳ患儿活动期红细胞免疫功能变化与激素治疗反应无关。结果提示单纯性和肾炎性ＮＳ存在不同的红细胞免疫功能改变，后者在ＮＳ发病机制中可能起重要作用。

Inordertostudythechangesofredcelimmunefunctioninchildrenwithprimarynephroticsyndrome(NS),wemeasuredtheredcelimmunefunctionin37casesofNSandobservedthechangesin28casesbeforeandaftertherapy.Theresultsshowedthat:(1)atactivestageofsim-pleNS,theerythrocyteC3breceptorroseterates(E-C3bRR)decreasedandtheerythrocyteimmuneadherenceinhibitingrates(EIAIR)increasedsignificantly.E-C3bRRinsimpleNSpatientsduringre-missionperiodwasstillowerthanthatofcontrolgroup.ThedecreaseinE-C3bRRinsimpleNSpa-tientswascloselyrelatedtothechangesofTcelsubsets;(2)atactivestageofNSwithnephritisfeatures,E-C3bRRandtheerythrocyteimmuneadherenceenhancingrates(EIAER)decreasedandtheerythrocyteimmunecomplexroseterates(E-ICR)andEIAIRincreasedsignificantly.Butalre-coveredduringremissionperiod;(3)therewasnorelationshipbetweenthechangesofredcelim-munefunctionatactivestageofNSandthedegreeofresponsetoprednisoneinpatientswithNS.TheresultssuggestthatthedeficiencyofredcelimmunefunctionplaysaroleinthepathogenesisofNS.
