595nm脉冲染料激光与1064nm长脉宽Nd-YAG激光治疗鲜红斑痣疗效对比

摘    要:目的回顾性分析比较595 nm脉冲染料激光及1 064 nm长脉宽Nd-YAG激光治疗鲜红斑痣的疗效及不良反应。方法应用595 nm脉冲染料激光(能量为8~15 J/cm^2,脉宽为1.5~10 ms)及1 064 nm Nd-YAG激光(能量密度为120~130 J/cm^2,脉宽25~45 ms)共治疗鲜红斑痣1 150例,从年龄、性别、皮损类型、不同治疗次数及不良反应等方面比较2种激光的疗效。结果 595 nm激光与1 064 nm Nd-YAG激光治疗鲜红斑痣有总效率分别为70.33%、76.99%,差异有统计学意义(χ~2=6.58,P<0.05);尤其是增厚型皮损,有效率分别为23.08%、58.47%,差异有统计学意义(χ~2=23.84,P<0.01);瘢痕发生率分别为0.37%、3.48%,差异有统计学意义(χ~2=14.16,P<0.01)。结论 595 nm脉冲染料激光及1 064 nm Nd-YAG激光治疗鲜红斑痣均安全有效,后者疗效略高于前者,但瘢痕发生率也略高。

Abstract:Objective In order to evaluate the efficacy and safety of 595 nm flashlamp-pumped pulsed dye laser and longpulse 1 064 nm Nd-YAG laser in treating patients with port-wine stains (PWS).Methods A retrospective review was performed in 1 150 patients with PWS who had been treated with 595 nm flashlamp-pumped pulsed dye laser (energy density:8~15 J/cm2;pulse width:10 ms) and long-pulse 1 064 nm Nd-YAG laser(energy density:120~130 J/cm2;pulse width:25~45 ms).The effects of two kinds of lasers were compared in terms of age,gender,skin types,different treatment times and adverse reactions.Results In the treatment of PWS with 595 nm laser and Nd-YAG laser,the total efficiency were 70.33% and 76.99%,the difference was statistically significant (x2=6.58,P<0.05).In thickening lesions,the efficiency were 23.08% and 58.47%,the difference was statistically significant (x2=23.84,P<0.01);the scar formation rates were 0.37% and 3.48%,the difference was statistically significant(x2=14.16,P<0.01).Conclusion The 595 nm flashlamp-pumped pulsed dye laser and long-pulse 1 064 nm Nd-YAG laser are safe and effective for the treatment of PWS.The curative effect of the latter is slightly higher than that of the former,but the incidence of scar is also slightly higher.
