摘    要:目的探讨IgE水平在不同类型冠心病中的意义。方法符合人选标准的患者按造影结果分为冠心病组(CHD,n=147)和非冠心病组(non-CHD,n=93),其中CHD组又分为稳定型心绞痛组(SAP,n=31)、不稳定型心绞痛组(UAP,n=83)和急性心肌梗死组(AMI,n=33)。用酶联免疫分析法测定血清IgE水平。结果CHD组血清IgE高于non—CHD组(99.55±9.84vs62.21±5.69,P〈0.01),多重线性回归模型提示仍有统计学意义(P〈0.05)。IgE水平(IU/mL):AMI组(133.63±26.28),UAP组(97.72±12.41),SAP组(68.18±15.76)。AMI组依次高于UAP组(P〉0.05)、SAP组(P〈0.01)和non—CHD组(P〈0.01)。UAP组依次高于SAP组和non—CHD组(P均〈0.05)。结论IgE是提示冠脉斑块稳定性的一个独立的生化指标,其可能在冠心病的发病中起着一定的作用。

The significance of serum IgE levels for different types of coronary atherosclerotic heart diseases

Abstract:Objective To establish a link between serum immunoglobulin E(IgE) levels and different stages of coronary atherosclerotic artery diseases.Methods All 240 enrolled patients were devided into CHD group(n = 147) and non-CHD group(n = 93) according to the results of angiography.The CHD group was subgrouped into those with acute myocardial infarction(AMI,n = 33) and those with unstable or stable angina pectoris(UAP,n = 83,and SAP,n = 31).IgE was measured by fluorescence enzyme immunoassay while other biomarkers were measured by enzymatic methods.Results Compared with non-CHD subjects,serum IgE levels were higher in CHD patients(99.55±9.84 vs 62.21±5.69 IU/mL,P < 0.01),it remained significantly higher in a multiple linear regression test after adjusting for all confounding factors(P < 0.05).IgE levels were nearly doubled in AMI patients as compared with non-CHD patients(133.63±26.28 vs 62.21±5.69 IU/mL,P < 0.01),and higher than in UAP patients(97.72±12.41 IU/mL,P > 0.05) or SAP patients(68.18±15.76 IU/mL,P < 0.01).UAP patients had significantly higher IgE levels than SAP patients(P < 0.05) or non-CHD patients(P < 0.05).Pearson’s correlation test revealed that serum IgE levels correlated significantly only with fasting blood glucose levels.Conclusion Our observations suggest that serum IgE is an independent biomarker for coronary plaque stability,which provides a new direction in further research of the mechanisms of atherosclerosis.
