外伤性骨盆骨折合并其他部位伤的诊治

目的：探讨外伤性骨盆骨折合并其他部位伤的诊治方法及最佳适应证。方法：对71例外伤性骨盆骨折合并其他部位伤的病例进行不同方法的诊治：脾切除16例，肝修补7例，结肠手术6例，小肠修补吻合6例，膀胱造瘘4例，髂内动脉结扎10例，半骨盆离断术1例，髂血管探查取栓4例，胸腔闭式引流5例。结果：全组死亡11例，死亡率15．5％。1例死于肺栓塞，1例死于ARDS，1例死于MOF，8例死于出血性休克。其余均治愈，治愈率84．5％。结论：在临床诊断抢救治疗时，应综合分析观察外伤性骨盆骨折合并其他部位伤病人的病程发展全过程，区别轻重缓急，有效救治，提高抢救的成功率。

The treatment and indication for traumatic pelvic fracture associated with injuries of other organ

Objective:To probe the treatment and indication for traumatic pelvic fracture associated with injuries of other organ. Methods:Of 70 cases with traumatic pelvic fracture associated with injuries of other organ,16 were treated by lienectomy,7 were treated by liver repair,6 were treated by colon operation,6 were treated by intestine inosculation,4 were treated by bladder fistulatome,10 were treated by ligation of internal iliac arteries,1 was treated by half-pelvic excision,4 were treated by angioembolization,5 were treated by conduction.Results: Totally 11 cases were dead,death rate was 15.5%.Of 11 death cases,1 died of pulmonary embolism,1 died of ARDS,1 died of MOF,7 died of shock.Conclusions: we should synthetically observe and analyze the whole course of traumatic pelvic fracture associated with injuries of other organ to improve the success ratio.
