两种正压通气治疗对阻塞性睡眠呼吸暂停综合征患者难治性高血压的疗效比较

目的比较持续气道正压通气（CPAP）和双水平正压通气（BiPAP）治疗对合并难治性高血压（RH）的阻塞性睡眠呼吸暂停综合征（OSAS）患者的疗效。方法用CPAP,BiPAP呼吸机分别治疗12例合并RH的中重度OSAS患者3个月,比较两种治疗方法前后的睡眠呼吸相关参数、血压以及血浆ALD、PRA水平。结果与治疗前相比,CPAP和BiPAP治疗3月时睡眠呼吸相关参数均较治疗前明显好转;受试者的血压均显著下降;晨起卧位ALD明显降低,但PRA无明显变化。结论 CPAP、BiPAP治疗均是无创治疗阻塞性睡眠呼吸暂停低通气综合征患者难治性高血压的有效措施。

Comparison of the efficacy between two positive pressure airway treatment to resistant hypertension in patients with obstructive sleep apnea syndrome

Objective To comparion the difference of the efficacy between continuous positive airway pressure（CPAP） treatment and Bi-level positive airway pressure（BiPAP） treatment in patients with obstructive sleep syndrome（OSAS） and resistant hypertension（RH）.Methods Twelve OSAS patients with RH were respectively treated with CPAP and BiPAP treatment.On three months＇ treatment,sleep apnea-related parameters,blood pressures and plasma concentrations of aldosterone（ALD） and plasma renin activity（PRA） were compared between before and after treatment.Results Compared with sleep apnea-related parameters before treatment,there was a significant improvement on three months＇ treatment with CPAP and BiPAP.Blood pressures and the morning plasma ALD at supine position were decreased significantly,but plasma PRA.Conclusion CPAP treatment and BiPAP treatment were both the efficient therapy in patients OSAS and RH.
