单倍体相合造血干细胞移植联合间充质干细胞输注对患者造血微环境的影响

摘    要:本研究探讨骨髓源间充质干细胞对单倍体相合异基因造血干细胞移植后患者造血微环境的影响。选择间充质干细胞联合造血干细胞移植的15名患者为研究组,单纯造血干细胞移植的20名患者为对照组,抽取35名患者移植后不同时期骨髓标本47份,进行MSC培养,观察MSC能达到原代融合的例数及其融合时间;从移植前开始抽取患者外周血以检测中性粒细胞和血小板的水平;每隔4天留取患者外周血浆标本,用ELISA检测SDF-1α、TPO及IL-11的浓度,观察两组之间浓度动态变化的差异。结果表明,研究组MSC体外原代融合率(60%)明显高于对照组(27.3%)(p0.01),且两组原代融合时间分别为27.13±4.41天与36.09±11.84天(p0.05)。回输组SDF-1α浓度在移植第8天达最高峰(2975.19±681.56pg/ml),较移植前(2403.70±522.39pg/ml)升高(p0.05),对照组SDF-1α浓度在移植后第16天达最大值(2280.60±701.25pg/ml),较移植前(2701.46±483.21pg/ml)减少(p0.05)。从移植后第16天至第28天,研究组TPO和IL-11的浓度均高于对照组(p0.05)。结论:联合间充质干细胞移植能明显改善单倍体相合异基因造血干细胞移植患者骨髓微环境的损伤状态。
