三种方法治疗翼状胬肉的疗效比较

目的探讨翼状胬肉单纯切除术、角膜缘干细胞移植术以及联合手术的临床特点及各自优势。方法选取2007年1月～2011年10月间在我院行单纯翼状胬肉切除术患者28例设为A组,另随机选择同期行角膜缘干细胞移植患者28例设立为B组,以及同期翼状胬肉切除联合自体角膜缘干细胞移植术患者28例设立为C组,比较三组的手术痊愈率及复发情况。结果各组患者均顺利完成手术,其中A组痊愈率71.9%,B组痊愈率85.3%,C组痊愈率87.1%,三组比较差异有统计学意义(P<0.05)。且B组、C组的痊愈率明显高于A组,差异有统计学意义(P<0.05)。A组术后复发率最高,达28.1%,B组、C组的复发率分别为14.7%、12.9%,两组比较差异无统计学意义,但二者均明显低于A组,差异有统计学意义(P<0.05)。但各组不同分级间复发率比较,差异无显著性。结论翼状胬肉切除联合自体角膜缘干细胞移植术能促进术区膜上皮修复,减少翼状胬肉复发,其手术效果明显优于翼状胬肉单纯切除术,是治翼状胬肉的一种行之有效的好方法,值得推广和应用。

The efficacy comparison of three methods in treatment of pterygium

Objective To investigate the pterygium excision alone,limbal stem cell transplantation as well as joint surgery clinical features and their respective advantages. Methods From January 2007 to October 2011 ,patients with simple pterygium excision in our hospital 28 was set to group A, another randomly selected the same time limbal stem cell transplantation in 28 cases as group B ,and the same period in wing-like pterygium excision combined with autologous limbal stem cell transplantation in 28 cases as the group C,compared three groups surgical cure rate and recurrence. Results Patients in each group were successfully completed surgery,group A recovered 23 ,the cure rate was 71.9%, group B recovered 29, the cure rate was 85.3% ,group C recovered 27 ,the cure rate was 87.1% ,the difference of cure rate was statistically signifi- cant in three groups （P 〈 0.05）. And the cure rate of group B,group C was significantly higher than group A,the differ ence was statistically significant （P 〈 0.05）. A postoperative recurrence rate of 28.1%, group B and C recurrence rates were 14.7%, 12.9% ,the two groups had no statistically significant differences,but both were significantly lower in group A, the difference statistically significant （P 〈 0.05）. Relapse rate between the various groups of different classification,the difference was not statistically significant. Conclusion The pterygium excision with limbal stem cell transplantation can promote epithelial repair surgery area,reducing pterygium recurrence,the effect of surgery was significantly superior to simple pterygium resection, a line of governance pterygium effective method worthy of promotion and application.
