软组织韧带样型纤维瘤病的影像诊断

目的分析不同部位软组织韧带样型纤维瘤病的CT和MR影像表现,提高该肿瘤的影像诊断水平。方法回顾性分析16例经病理确诊的软组织韧带样型纤维瘤病的影像学表现,观察病变的位置、大小、边缘、形态、密度/信号特点、强化类型。结果肿瘤位于腹壁6例,体积小,形态规则,与肌肉分界清;肿瘤位于腹壁外10例,体积大,形态不规则,与肌肉分界不清。所有病灶MRI均表现为肌肉内占位性病变,T1WI呈等信号、稍低或稍高信号,T2WI高于肌肉低于脂肪,T1WI、T2WI内见条带状低信号影。无论肿瘤发生在什么部位,病灶内都没有囊性坏死区。结论CT、MR可以较为精确地显示软组织韧带样型纤维瘤病的病灶部位、形态以及范围,对该病的诊断及鉴别诊断具有重要价值。

Imaging diagnosis of soft tissue desmoid-type fibromatosis

Objective To Analyze CT and MRI manifestations of soft tissue desmoid-type fibromatoses in different parts to improve diagnostic accuracy. Methods Imaging manifestations of 16 cases of patients with desmoid-type fibromatoses confirmed by surgical pathology were reviewed and analyzed retrospectively. The location, size, boundary, shape, density/signal intensity, type of enhancement of theses lesions were observed. Results Six of 16 fibromatoses were located in abdominal wall muscles had small size and regular shape, showed well-defined borders with surrounding muscles. Another 10 in extra-abdominal were relatively large and irregular, appeared ill-defined borders. All fibromatoses were showed occupied tumors in muscles On MRI, Heterogenous isointensity, slight hypointensity or mild hyperintensity on T1WI, mild hyperintensity on T2WI were seen, which lower than signal of fat and higher than that of muscles, and all cases appeared low linear signal intensity on both T1WI and T2WI.No Cystic necrosis were found in all cases no matter its located in anywhere. Conclusion The CT and MRI can accurately show the location, shape and extent of desmoid-type fibromatoses, which is valuable in differential diagnosis of various causes.
