摘 要: | 目的探讨甲胎蛋白(AFP)、α-L岩藻糖苷酶(AFU)、糖类抗原19-9(CA19-9)和γ-谷氨酰转肽酶(GGT)在原发性肝癌诊断中的临床价值。方法对我院就诊的30例原发性肝癌患者、40例肝炎、肝硬化、40例健康对照组血清AFP、AFU、CA19-9和GGT的浓度及阳性率进行检测,并对比分析。结果 AFP、AFU、CA19-9和GGT在原发性肝癌的浓度均明显高于肝炎、肝硬化组及正常对照组(P<0.01)。而AFP、AFU、CA19-9和GGT在肝炎、肝硬化组的浓度也明显高于正常对照组(P<0.05)。AFP、AFU、CA19-9和GGT在原发性肝癌组织中的阳性率明显高于肝炎、肝硬化组的阳性率(P<0.05)。但在正常组织中的阳性率均为0。且AFP、AFU、CA19-9和GGT四项指标联合检测阳性率明显高于任意指标单独检测的阳性率(P<0.05)。AFP阳性率随着肿瘤组织的直径增大,其阳性率明显增加,其他未见明显差异。结论 AFP、AFU、CA19-9和GGT联合检测可提高原发性肝癌诊断的阳性率,对原发性肝癌的早期诊断治疗和预后均有重要的临床应用价值。

Abstract: | Objective To explore the clinical value of alpha-fetoprotein (AFP), α-L fucosidase (AFU,), carbohydrate anti- gen 19-9 (CA19-9) and γ-glutamyl acyl transfer peptidase (GGT) in the diagnosis of primary liver cancer. Methods Se- lected 30 patients with primary liver cancer in our hospital, 40 cases of hepatitis, liver cirrhosis, 40 cases of healthy control group, and the serum of AFP, AFU, CA19-9 and the concentration and positive rate of GGT were compared and analyzed. Results AFP, AFU, CA19-9 concentration in primary liver cancer and GGT were significantly higher than those in hepatitis, liver cirrhosis group and the normal control group (P 〈 0.01 ). AFP, AFU, CA19-9 and GGT concentrations in the group of hepatitis, liver cirrhosis group were significantly higher than those in normal control group(P 〈 0.05). AFP, AFU, CA19-9, and GGT in primary hepatocellular carcinoma, the positive rate was significantly higher than that in hepatitis, cirrhosis of the group (P 〈 0.05). But the positive rate was 0 in normal tissue. And the positive rate of AFP, AFU, CA19-9 and GGT detected was significantly higher than that of the separate detection of any indicators (P 〈 0.05). With the increase of the tumor diameter, the positive rate of AFP increased significantly and the other was no significant differ- ence. Conclusion AFP, AFU, CA19-9 and GGT joint detection can improve the positive rate of diagnosis of primary liver cancer. It has important clinical value of primary liver cancer early diagnosis and treatment and prognosis. |
