妊娠期糖尿病患者实施一体化系统管理的效果

目的探讨妊娠期糖尿病患者实施一体化系统管理的效果。方法将符合纳入标准的妊娠期糖尿病患者分实验组(83例)和对照组(127例),实验组由专职医护人员进行4阶段一体化系统管理(即门诊集中授课;制订个体化饮食方案,建立精神支持;加强医护人员、患者、家属的管理;评估认知状况);对照组按妊娠期糖尿病进行常规护理。结果实验组妊娠期糖尿病患者妊娠高血压疾病、羊水过多、胎膜早破、早产的并发症发生率低于对照组,差异具有统计学意义(P0.01或P0.05);实验组围生儿出现巨大儿、新生儿低血糖的并发症发生率均低于对照组,差异具有统计学意义(P0.01或P0.05)。结论妊娠期糖尿病患者实施一体化系统管理,可有效减少母婴并发症,值得临床推广应用。

Effect of management of the integration system on patients with gestational diabetes mellitus

Objective To explore the effect of management of the integration systery on patients with Gestational Diabetes Mellitus （GDM）. Methods The patients with GDM meeting the inclusion criteria were divided into the experimental group and the control group. Patients in the experimental group were received professional medical staffg four stage management system ,which included centralized lecture in outpatient department, developing individualized diet plans and spir- itual support ,enhancing the management of medical staff,patients and patients＇caregivers ,assessing cognitive status. Patients in the control group were received rou tine nursing care. Results Multiple index of the prognosis of patients with diabetes during pregnancy in experimental group were lower than the control group, such as pregnancy induced hypertension, polyhydramnios, premature rupture of membranes and preterm birth （P 〈 0.01 or P 〈 0.05 ）. The complications in experimen- tal group, such as macrosomia and neonatal hypoglycemia were lower than the control group （P 〈 0.01 or P 〈 0.05 ）. Conclusion The implementation of manage ment of integration system on patients with GDM can significantly reduce the incidences of maternal and neonatal complications ,which is worth spreading.
