闭合性腹部外伤的诊疗体会

目的:总结闭合性腹外伤的诊治经验,进一步提高救治水平。方法:回顾分析112例闭合性腹部外伤患者的诊治情况。结果:保守治疗6例,行剖腹手术106例。103倒经手术治疗抢救成功,抢救成功率为97.17%;2例在术中分别死于失血性休克和多脏器功能衰竭,1例术后2d死于多脏器功能衰竭。结论:腹部外伤表现复杂。结合病史、体格检查、腹腔穿刺、B超、CT、X线检查等可提高诊断率,为腹部外伤治疗提供可靠依据。手术止血是最根本的治疗措施,术中探查应避免遗漏脏器损伤。正确无误的诊断和正确处理合并伤是减少并发症、降低病死率、提高救治效果的关键。

The diagnosis and treatment experiences of closed abdominal injury

Objective To summarize the experiences of diagnosing and treating closed abdominal injury,So as to further improve the curative levels.Methods The diagnosis and treatment of 112 patients with closed abdominal injury were retrospec- tively analyzed.Results of the 112 patients with closed abdominal injury,6 cases received conservative treatments and 106 ca- ses underwent laparotomy;103 cases were cured but 3 cases died of hemorrhagic shock,infectious shock,multiple organ dys- function syndrome and severe associated injury.Condusion The correct diagnostic rate can be improved and the reliable evi- dence can be provide for the treatment of abdominal injury if the disease history,physical examination,abdominal paracentesis, B mode ultrasound,CT can be combined.Surgical hemostasis is the most basic therapeutic measure,and perioperative probing can avoid the omission of organ injury.The correct treatment of associated injury is a major factor for reducing complications and decreasing fatality rate.
