6例双原发癌临床分析

目的:通过对2011年1月至2012年10月期间郑州大学附属肿瘤医院普外科6例双原发癌患者的诊断及治疗的临床研究,认识到提高术前诊断率及手术治疗的重要性。方法:对6例双原发癌患者进行回顾性分析,3例经胃镜检出第一癌,手术探查发现第二癌;1例经结肠镜发现第一癌,CT发现且经纤维支气管镜确诊第二癌;2例经胃镜发现第一癌,穿刺活检发现第二癌;其中4例手术治疗+化疗,1例单纯化疗,1例手术治疗+放疗。结果:5例手术切除肿瘤,1例拒绝手术,术后顺利恢复,均达临床治愈。结论:提高双原发癌的术前诊断率,完善术前准备,及时行手术治疗,从而减少手术并发症,术后辅以放化疗,能提高治疗效果。

Clinical analysis on 6 cases of double primary carcinomas

Objective: To recognize the importance of improve preoperative diagnosis and surgical treatment of the clinical study of six cases of double primary cancer diagnosis and treatment.Methods: Six cases of double primary cancer were retrospectively analyzed,and three cases of the first cancer detected by endoscopy,surgical exploration found a second cancer.One case was found by colonoscopy cancer,CT discovered and diagnosed with a second cancer by fiberoptic bronchoscopy.Two cases were found of the fiirst cancer by endoscopy and which were found of the second cancer by the biopsy.The treatments of 4 cases were surgical treatment and chemotherapy.One case was chemotherapy,and the remaining one case was Surgery and radiotherapy.Results: Among all the 6 double primary carcinomas cases,five cases were removed the tumor by surgery,one patient refused surgery.All of them recovered smoothly postoperative and reached to the clinical cure.Conclusion: If we could improve the preoperative diagnosis rate of double primary cancer,perfect the preoperative preparation,and did timely surgical treatment,we can reduce surgical complications.If we could do the adjuvant chemotherapy after the surgery,we can improve the treatment.
