心脏起搏治疗中血浆B型尿钠肽的变化

目的通过对43例永久起搏治疗患者手术前后血浆B型尿钠肽(BNP)水平的变化,分析心脏起搏治疗对BNP的影响。方法收集2003-03~2005-01在本院因病态窦房结综合征(SSS)或房室传导阻滞(AVB)行心脏起搏治疗的43例患者,其中AAI模式3例,DDD 13例,VVI 27例,并在手术前后分别测定血浆BNP的水平。结果AAI患者BNP水平无明显变化,DDD患者BNP水平轻度下降,无统计学意义(P=0.44)。VVI患者中不伴室房逆传者BNP水平轻度升高,无统计学意义(P=0.08),VVI患者中伴室房逆传者BNP水平明显升高(P<0.05)。结论血浆BNP水平不仅与心室的张力有关,还与心脏的内分泌、传导顺序等密切相关。

The Change of Plasma B-type Natriuretic Peptide(BNP) after Installing of the Permanent Pacemaker

Objective To explore the change of plasma BNP after installing of the permanent pacemaker.Methods We collected 37 patients with sick sinus syndrome(SSS) or atrioventricular block(AVB).3 subjects with AAI mode,13 subjects with DDD mode,27 subjects with VVI mode.We measured plasma BNP levels pre-and after-installing permanent pacemaker,respectively.Results The plasma BNP levels unchanged in the AAI.The plasma BNP levels descended lightly in the DDD(P=0.44).In the VVI without ventriculo-atrial conduction,the plasma BNP levels increased lightly(P=0.08),but it increased evidently with ventriculo-atrial conduction(P<0.05).Conclusion The results suggest the change of plasma BNP is correction with ventricle strain,cardiac hormone,and the sequence of conduction.
