射频消融治疗右侧旁道

目的总结射频消融右侧旁道的经验。方法回顾性分析22例右侧旁道患者的射频消融经过和结果。结果22例患者中首次消融成功20例,成功率91％,复发2例,复发率10％。首次消融未成功的2例中1例二次手术后成功;1例右侧显性旁道伴房间隔缺损,反复发作室上速和房颤,首次消融未能成功,术后仍然反复发作,并突发大面积脑梗塞,家属放弃继续治疗,总成功率99％。结论提高对靶点图的识别能力和消融导管的稳定性,仔细认真操作,右侧旁道的消融将会更安全有效．

Radiofrequency catheter ablation(RFCA) in the treatment of supraventricular tachycardia with Right-side pathway

Objective To summarize the experience of radiofrequency catheter ablation （RFCA） in the treatment of right-sided accessory pathway. Methods The effect and results of RFCA in 22 patients were retrospective analyzed. Results Among them, 20 cases （91%） were ablated successfully in the first procedure, 2 pa- tients relapsed. Recurrence rate was 10%. The other two patients who were failed in the first process. One of them ablated successfully by second procedure, the other one, who was right accessory atrioventricular pathways with a- trial septal defect, at the same time, repeated attack atrial Ftbrillation and supraventricular tachycardia were recurrent after operation and sudden onset of massive cerebral infarction. Total success rate of RFCA was 99%. Conclusion Improving the ability of recognizing targeted electrogram, keeping the catheter stable and operating earnestly during the operation are the keys to perform succeed in RFCA in patients with right-sided accessory pathway.
