氧气驱动雾化吸入肾上腺素在减轻新生儿机械通气拔管后喉水肿中的应用

摘    要:目的观察氧气驱动吸入肾上腺素对减轻新生儿机械通气拔管后喉水肿的疗效。方法88例机械通气的新生儿随机分为两组,对照组43例,在拔管前1h静脉注射地塞米松0.5mg/kg,拔管后予以地塞米松2mg加生理盐水2ml,用6L/min氧气驱动雾化吸入,2h1次。治疗组45例,拔管后立即予以肾上腺素1mg加生理盐水2ml,用6L/min氧气驱动雾化吸入,2h1次,雾化4~5次后改用常规雾化吸入。两组均予以吸氧、吸痰、抗感染等处理。结果治疗组总有效率86.6%,明显优于对照组(总有效率67.4%)(P<0.05)。结论氧气驱动雾化吸入肾上腺素对减轻新生儿机械通气拔管后喉水肿效果明显,值得临床推广应用。

Application of oxygen-driven aerosol inhalation of adrenaline to alleviate neonates''''laryngeal edema after decannulating their mechanical ventilators
Abstract:Objctive To study the clinical effects of oxygen-driven aerosol inhalation of adrenaline on the neonates′ laryngeal edema after removing their mechanical ventilators. Methods Eighty-eight cases were randomly divided into two groups, control group (n=43) treated with dexamethasone, in a dosage of 0.5 mg/kg every time by intravenous injection 1 hour before stopping mechanical ventilation, and then treated with 6 L/min oxygen-driven aerosol inhalation of dexamethasone, adding 2 mg dexamethasone to 0.9% sodium chloride (2 ml) every time, once every 2 hours. Treatment groups (n=45) treated with 6 L/min oxygen-driven aerosol inhalation of adrenaline, adding 1 mg adrenaline to sodium chloride (2 ml) each time, once every 2 hours. Having done this for 4 to 5 times, the patients were treated with conventional nebulization. Both groups were treated with routine therapy, including anti-infective treatment, oxygen inhalation,sputum aspiration etc. Result The total effective rate was 86.6% in treatment group, much better than that in control group (67.4%) (P<0.05). Conclusion Adrenaline inhaled via oxygen-driven atomizer is an effective therapeutic method to alleviate neonates′ laryngeal edema after removing their mechanical ventilators. It is worth spreading in clinical use.
