磁共振胰胆管成像在胰胆管疾病中的诊断价值

目的:探讨磁共振胰胆管成像(magnetic resonance cholangiography,MRCP)对胰胆管疾病的诊断价值。方法:用快速自旋回波(TSE)、半傅立叶采集单次激发快速自旋回波(HASTE)和常规T1加权快速小角度激发梯度回波(FLASH)序列完成169例MRCP检查,其中74例经内窥镜逆行胰胆管造影(ERCP),95例经手术病理证实,其余为B超和CT证实。结果:全部检查均获得成功。MRCP技术结合B超和CT图像对胰胆管疾病的早期诊断和鉴别诊断具有十分重要的价值。良、恶性胰胆管疾病在MRCP图像上具有一定的特征性。MRCP定位及定性准确率分别为100%和95.8%。结论:MRCP是无创性的有效检查手段,能清楚显示正常或异常的胰胆管结构,定位和定性准确率高,能为临床提供十分有意义的诊断资料。

The value of MR cholangiopancreatography in diagnosing pancreaticobiliary duct diseases

Objective:To evaluate the diagnostic value of MR Cholangiopancreatography in diagnosis of pancreaticobilitary duct diseases.Methods: MRCP was performed in 169 cases with fast acquisition interleaved spin echo(TSE) and half Fourier acquired single shot turbo spin echo(HASTE) and normal Weighted T1 fast low angle gradient echo.In this data,74 cases were verified with ERCP,95 cases were verified with operation and histology,and the rest cases with CT or ultrasounography.Results:MRCP were satisfactorily performed in all cases.MRCP with CT and ultrasounography images on valuable of the cholangiopancreatic disease in the early diagnosis and differential diagnosis.The malignant or benign diseases of pancreaticobiliary duct may be shown characteristic appearances in MRCP.The accuracy of MRCP for evaluating the site and the causes of these diseases were 100% and 95.8% respectively.Conclusion:MRCP is a noninvasive technique for depicting the normal and abnormal pancreaticobiliary duct,and it had the excellent,accuracy in the determination of the locations and disease.
