端粒酶活性联合p16启动子超甲基化在胆囊癌中的表达意义

摘    要:目的探讨胆囊癌组织和癌旁组织中端粒酶活性和p16基因启动子超甲基化的意义。方法采用TRAP-ELISA和MSP检测胆囊癌及癌旁组织各46例中端粒酶活性和p16基因启动子区超甲基化。结果46例胆囊癌中33例(72%)p16超甲基化阳性,癌旁组织有14例(30%)p16超甲基化,两者有明显差异(P<0.05)。46例胆囊癌中31例(67%)端粒酶阳性,而癌旁组织端粒酶阳性仅有4例(9%),两者有显著性差异(P<0.05)。p16超甲基化和端粒酶阳性率这两项指标在胆囊癌的不同分化程度中无明显差异(P>0.05);中、低分化胆囊癌与高分化胆囊癌端粒酶积分光度比较有显著性差异(P<0.05)。胆囊癌p16甲基化和端粒酶检测一致性较好(P<0.01)。结论联合检测端粒酶活性与p16启动子异常甲基化对胆囊癌的诊断有一定价值。

Abstract:Objective It is to approach the meaning of expression of telomerase activity combined with hypermethylation of p16 promoter in gallbladder carcinoma tissues and adjacent non-tumor area.Methods The TRAP-ELISA and MSP were applied to detect telomerase activity and hypermethylation of p16 promoter in each 46 cases of gallbladder carcinoma tissues and adjacent non-tumor area.Results Hypermethylation was found in 33(72%) tumor tissues and 14(30%) adjacent non-tumor tissues from the each 46 tissues,there were obviously difference between the two groups(P<0.05).And the telomerase positive was detected in 31(67%) tumor tissues and 4(9%) adjacent non-tumor tissues,there were obviously difference between the two groups(P<0.05).There were no difference both of p16 hypermethylation and telomerase activity related to histological differentiation of gallbladder carcinoma(P>0.05);there were obviously differences between the high differentiated and the mediate or low differentiated carcinomas(P<0.05).There was high concordance of p16 hypermethylation and telomerase activity in measurement by Kappa analysis(P<0.01).Conclusion It is valuable to detecting telomerase activity combine with hypermethylation of p16 promoter for the diagnosis of gallbladder carcinoma.
